How to Start Hospice Care
Anyone can refer a patient to hospice, even patients themselves.
Although, eligibility for hospice service must be certified by the patient’s attending physician and the hospice medical director.
Hospice Eligibility – A diagnosis with a life-limiting condition and prognosis of 6 months or less, should the disease follow its natural course.
Those with illnesses in which curative measures are no longer effective, appropriate, or desired.

Determining the level of care
The hospice physician will determine what levels of care are needed using standardized criteria.

Hospice Levels of Care
There are four levels of hospice care defined by law and reimbursed for by Medicare and most other insurance plans. Hospice care is available regardless of the patient’s geographical location and keeps patients in the setting that best meets their needs.

ROUTINE CARE
The patient receives hospice services at home or in a home-like setting. The family provides primary care to the patient with the assistance of the hospice team.

CONTINUOUS CARE
Skilled nursing services are provided in the patient’s place of residence to help during a crisis period.

INPATIENT CARE
This care is provided in a facility for symptoms or a crisis that cannot be managed in the patient’s residence. Inpatient care is provided for a limited period of time, as determined by the physician and the hospice team.

RESPITE CARE
This service is designed to give caregivers a rest. Respite care is up to five days and nights at a time and is provided in a skilled nursing facility.

Important Note

Not every level of hospice care provides 24-hour supervision. The goal of hospice is to empower the family by providing the needed support to care for their loved ones.
